APPENDIX A NOTICE OF VIOLATION T
Power Authority of the' State of New York Docket No. 50-333 FitzPatrick Nuclear Power Station License No. OPR-59 As a result of the inspection conducted on September 1-30, 1981 and in accord-ance.with the Interim' Enforcement Policy, 45 FR 66754, (October 1, 1980), the following violation was identified:
A.
Technical Specification 6.8(A) states in part:
" Written procedures and administrative policies shall be established, implemented..."
Operating Procedure F-0P-22, Diesel Generator Emergency Power, Revision 4, dated June 26, 1980, paragraph C.2 status in part, " Ensure doors between DG rooms are closed when the DG's are required to be operable.
Contrary to the above, at 2:20 p.m., on September 18, 1001 and at 3:50 p.m., on September 29, 1981 the inspector found door DG 272/2 between the diesel generator switchgear rooms open while the reactor was operating at power and the diesel generators were required to be operable.
This is a Severity Level V Violation (3upplement I).
Pursuant to the provisions of 10 CFR 2.291, The Power Authority of the State of New York is hereby required to. submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results Schieved;.(2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted i
under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
