APPENDIX A NOTICE OF VIOLATION i
Florida Power Corporation Docket No. 50-302 Crystal River 3 License No. DPR-72 As a result of the inspection conducted on April 27 - May 1,1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified.
A.
10 CFR 20.103 requires the use of measurements (surveys) of concentrations of radioactive materials in air for timely detection and assessment of individual intakes of radioactivity by exposed individuals.
Contrary to the above, a survey or surveys were not performed on March 31, 1981, when work was performed in the hot machine shop which required respiratory protection and involved a potential airborne radioactivity hazard. Workers were decontaminating tools at the time.
This is a Severity Level V Violation (Supplement IV.D.1).
B.
Technical Specification 6.11 states that procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure.
Radiological Control Procedure RP-102, Revision 12, entitled Respiratory Equipment Manual states:
(1) in Section 8.1.4, that self-contained breathing apparatus will be inspected monthly with the following points being ascertained:
a.
Air cylinders are charged to a minimum value of 1800 psig.
b.
Facepiece assemblies are totally functional and properly stored.
(2) in Section 10.1.2, that every compressed gas cylinder for respiratory use shall have a LABEL indicating that it contains pure breathing air, or pure breathing oxygen, as appropriate.
(3) in Section 8.2.1.1, that cylinders will be tested ro cinely in accord -
ance with the manuf&cturer's instructions. Instructions published by the manufacturer indicate that cylinders must be hydrostatically tested every five years.

Docket No. 50-302 Crystal River 3 License No. DPk-72 Contrary to the above, (1) On April 28, 1981, one Self Contained Breathing Apparatus (SCBA) on the Turbine Builcing 95' elevation was available for use yet was low in air pressure (approxima ely 1700 psig);
(2) On April 28, 1981, two of six SCBA spare cylinders which were available for use were at less than 1800 psig; (3) On April 28, 1981, a SCBA unit in the Auxiliary Building was found to have an improperly stored facepiece 'not in a sealed bag) yet was available for use; (4) On April 28, 1981, SCBA cylinders were found to have no distinctive LABEL indicating contents as required; (5) On April 28, 1981, one SCBA cylinder was found to have been last tested hydrostatically 'n 1974 This is a Severity Level V Violation (Supplement IV.E.2).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the cate of this Notice, a written statement or explanation in reply, including: (1) acmission or cenial of the a'leged viola-tions; (2) the reasons for the violations if anmitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration mey be given to extending your response time for good cause shown.
Under the authcrity of Sectica 182 of the Atomic Energy Act of 1954, as amended, this responsa shall be submitted under oath o affirmation.
