RESPONSE TO VIOLATION NUMBER 416/81-22-02

" FAILURE TO PROVIDE QUALITY ASSURANCE INDOCTRINATION"

1)

Admission or Denia' of the Alleged Violation:

The Company acknowledges the violation and admits that maintenar.ce

technicians (in this case contracted maintenance technicians) were

performing safety-related work without documented quality assurance

training.

2)

The Reason for the Violation:

The company did not follow procedures.

3)

Corrective Steps Which Have Been Taken & the Results Achieved:

a.

Intensified QA indoctrination training sessions were immediately

conducted to ensure that all personnel who perform safety-

related work were properly trained. To date, approximately 95%

of all GGNS personnel (including contracted maintenance

technicians) have participated in QA indoctrination.

b.

Supervisors are maintaining current lists of personnel under

their control with the status of their QA indoctrination

identified.

c.

All Supervisors have been directed, by memorandum from the Plant

Manager, to ensure that personnel identified as not having

participated in a QA indoctrination shall not independently

perform safety-related tasks at GCNS.

4.

Corrective Steps Taken to Avoid Future Violations:

Genersl Employee Training was formally implemented in March, 1981.

The QA indortrination was incorporated into the General Employee

Training on June 23, 1981. Procedural controls have been developed

which require completion of the General Employee Training (including

QA indoctrination) prior to performance of independent safety-related

work.
