Event Narrative:

Unit 1 was operating at 98-percent power, unit 2 was in a refueling outage, and unit 3 was operating at 98-percent power. Only unit 2 was affected by the event.

During the daily source and background check of continuous air monitor (CAM) 2-90-250, personnel discovered the. charcoal filter was not in place. This caused l

the CAM to operate in a degraded mode (T.S. 3.8.B.6).

Last authorized removal of the filter was at 1240 on October 3,1982 during routine perfomance of background and source check procedure. Therefore, the filter must be assumed to have been out of the CAM for a maximum 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />. The halogen channel was still capable of detecting iodine, though in a less sensitive manner.. A review of SI 4.8.B.1.a.1 indicated no significant releases for the 2-90-250 CAM or for any of the other CAM's during this period. There was no effect on public health and safety. There are no redundant systems. The cause of this event was personnel error. The event has been discussed with the individual involved..Upon discovery the filter was replaced. This is considered an isolated occurrence and.

no further recurrence control is required.
