C HIGHLIGHTS OF WEEKLY INFORMATION REPORT Week Ending May 17, 1991 Fire Event Update As a result of the main transformer short circuit and subsequent fire underneath the main generator un April 29, 1991, an Augmented Inspection Team (AIT) was dispatched to Maine Yankee Atomic Power Station (Maine Yankee). The AIT was chartered with verifying the circumstances and evaluating the significance of the subject event.

At the conclusion of their on-site inspection activities, the AIT conducted an exit meeting May 10, 1991, that was open to the public. The meeting was attended by many members of the public and news media.

Preliminary AIT findings were consistent with accounts available in local and national media (i.e.,

short circuit in the main transformer, inmediate turbine-generator trip and reactor scram, arcing caused a hydrogen fire under the generator that was promptly brought under control, no unusual effect on the reactor or its auxiliaries, no unusual radioactive release to the environment, approximately 150 gallons of PCB-free transformer oil washed into Back River by automatic initiation of transformer water deluge system).

The AIT had a generally favorable impression of Maine Yankee's actions in this matter. The AIT report currently is being drafted and will be made available to the public upon completion.

Although the AIT concluded that Maine Yankee correctly carried out its notification procedure, the Federal Emergency Management Agency (FEMA) is reviewing the entire chain of offsite communications associated with this event.

FEMA has agreed to inform NRC of its findings.

Pilgrim - Maine Yankee - Seabrook - Vermont Yankee Congressman Kostmayer will visit these four facilities during the May 25-28 period. Regionai management and OCA will escort the Congressman and two congressional staffers. He will meet wi.:1 the SRIs, licensee representatives, and tour the facilities.

Fort St. Vrain Spent Fuel Shipments On May 8, 1991, Public Services Company of Colorado (PSC) provided notification to NRC for shipment of spent reactor fuel from Fort St. Vrain to the Department of Energy's Idaho National Engineering Laboratory.

Subsequently, Governor Andrus appealed the case to the 9th Circuit Court and was granted a stay forbidding transport pending resolution of the case.

Palo Verde Unit No.-1, 2 and 3 Jury selection for-the trial involving Earth First!, is scheduled to begin on June 4, 1991, in Prescott, Arizona. Several Earth First! members are charged with conspiracy to down power lines leading to Palo Verde Diablo Canyon and Rocky Flats. The trial is expected to begin in mid to late June 1991, and last 2 to 3-months.

On April 22-23, 1991, OGC and NRR staff members, and one consultant from Brookhaven met with the U.S. Attorney's Office to discuss the trial.

As a result of motion filed by the U.S. Attorney and approved by the court, the participation by the NRC will be more limited than originally anticipated.

Current plans

-include questions concerning the licensing process and the layout of both the Diablo Canyon and Palo Verde sites.

2 Senior Project Manager for Palo Verde, Mr. Charles Tranmell may becone the principle NRC witness. Mr. Trammell has a detailed knowledge of the licensing process of both the Palo Verde and Diablo Canyon s'tes. Another meeting will take place later this month with the U.S. Attorney's Office to review the scope of the questions to be asked at the trial.

Agenda for OECD/CNRA Meeting on Shutdown / Low Power issues On May 9-10, 1991, W. T. Russell, DST staff, and their contractor (SAIC)

_ participated in meetings with members of an ad hoc OECD/CNRA working committee on Shutdown / Low Power issues. During these meetings, the working committee reviewed the responses of member countries to a questionnaire on 7

the state of the art in regulation of shutdown and low power operations and developed an agenda for the OECD/CNRA delegates meeting to take place in Paris on June 25, 1991.

The U.S. (NRR/ DST) has the lead for technical coordination of CNRA activities regarding shutdown / low power _ issues and prepared a draft discussion paper on the responses to the questionnaire which facilitated the committee's deliberations. The agenda developed by the working committee for the June 0 ECD /CNRA meeting includes the following presentation / discussion topics and discussion leaders:

Requirements, operating practices, recovery actions, training and systems for mitigation associated with assuring the principal safety functions of reactivity control (France / Canada), inventory control (Germany), decay heat removal (USA), and containment integrity (Spain);

2.

Planning and control of plant outages (Sweden);

3.

Regulatory actions to date regarding safety during shutdown and low power operations (Canada / France / USA);

Sponsored Symposium on Testing of Pumps and Valves (EMEB)

On May 13, 1991, the Pump and Valve Section of the Mechanical Engineering Branch participated in a meeting with members of the ASME Operations and Maintenance (0&M) Subcommittee on Performance Testing to establish the groundwork for a jointly sponsored symposium on testing of pumps and valves.

The symposium is scheduled for May 19-21,_1992, to be held in the Washington, D.C. area. Topics in four major areas were proposed:

(1) motor operated vahe testing, (2) inserv!ce testing programmatic issues, (3) inservice testing +echnical issues, and (4) regulatory / codes and standards issues. A i

" call for papers" and an announcement of the symposium will be issued in the near future. Additional planning meetings will be conducted periodically to finalize meeting logistics and to ensure that other activities are progressing on schedule. Several actions were assigned to get efforts underway.

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending May 17, 1991 Fort St. Vrain Spent Fuel Shipments On May 8, 1991. Nuclear Fuel Services-(NFS) Plutonium Building Decommissioning On May 14, 1991, a meeting was held with members of the NFS Erwin staff to discuss safeguards to be employed during the decommissioning of the plutonium building.

The Department of Energy (DOE) has advised NFS that the recovered plutonium nitrate must be converted to the oxide form before it can be shipped to Oak Ridge for recovery or storage, and the size of each shipment is to be limited to a relatively small amount. These constraints are causing NFS to reconsider safeguards procedures for handling and storage of the recovered scrap and waste.

Revised safeguards plans will have to be submitted for staff review.

Safeouards Agreement Working Group (SAWG) Meeting On May 14, 1991, an interagency SAWG meeting was held at the Department of State to discuss items that will be brought up at the June 17-19, 1991, U.S./ International Atomic Energy Agency (IAEA) Safeguards Implementation Review meeting. The following issues were of particular interest:

IMNS expects to publish the environmental assessment for the license renewal later this year.

Fire Safety Training Training in the use of a newly issued fire safety inspection procedure for fuel cycle and major materials facilities was provided to regional inspectors and supervisors on May 6-8, 1991. The training was given by the Nuclear Material Safety and Safeguards fire safety engineer at the NRC Technical Training Center.

Fuel Cycle Seminar On May 9-10, 1991, Nuclear Material Safety and Safeguards and Region 11 staff sponsored a Fuel Cycle Seminar in Atlanta, Georgia.

The seminar which was open to the public had about 120 attendees.

NRC representatives provided information on NRC policies and procedures applicable to the fuel cycle licensees, and updated pending issues and policy matters. The topics included nuclear criticality safety issues, the status of new Part 20, NRC enforcement and investigation issues, decommissioning, and licensing and safeguards issues.

Fuel cycle licensees participated in presentations on criticality safety and decommissioning.

Representatives of fuel cycle licensees also discussed their concerns in complying with NRC regulations, and provided their views on existing and pending NRC issues.

Department of Veteran's Affairs (VA) Meeting The NRC will be assisting the Department of Veteran's Affairs (VA) in a Radiation Safety Course for VA Broad Scope Radiation Safety Officers, May 30-June 1, 1991, in Little Rock, Arkansat.

Dr. John E. Glenn and Dr. Donna-Beth Howe from NMSS's Medical, Academic and Commercial Use Safety Branch and Gary Sanborn and Linda Kasner from Region IV will be providing lectures on NRC's licensing, inspection, and enforcement policies.

U.S. EPA Region Vil Re3resentatives to Visit NRC Headquarters for Review of West Lake Landfill Documents Two representatives from the Environmental Protection Agency's (EPA'S) Region Vil offices in Kansas City, Kansas, plan to visit with staff members of the Division of Low-Level Waste Management and Decommissioning on May 22-23, 1991, to review agency records concerning the West Lake Landfill near St. Louis, Missouri. This site was used by Cotter Corporation, a former Atomic Energy Commission licensee, to dispose of 43,000 tons of radioactively contaminated soil in 1973.

In August 1990, West Lake Landfill was listed on the CERCLA National Priorities List for Uncontrolled Hazardous Waste Sites.

EPA Region Vil representatives will acquire information from the Nuclear Regulatory Commission docket files to support preparation of their report on potentially responsible parties involved in the West Lake Landfill.

A remedial investigation / feasibility study for this site is planned, but not yet scheduled by EPA Region Vll.

ENCLOSURE B May 17, 1991

NRC/ EPA RCRA Training Workshops The Division of low-Level Waste Management and Decommissioning (LLWM), in conjunction with the Environmental Protection Agency (EPA), the Technical Training Center (TTC), and ICF, Inc. has developed a one-day workshop to f amiliarize Nuclear Regulatory Comission (NRC) personnel on the requirements of the Resource Conservation and Recovery Act (RCRA) and the management of mixed waste.

To date, this workshop has been presented in Regions 11 and Ill.

The workshop is scheduled to be presented in Regions 1, IV and V and the Uranium Recovery Field Office later this month. LLWM is also planning on presenting the workshop for NRC Headquarters personnel on July 9 and July 18, 1991. Supervisors who have employees that could benefit from the workshop have been instructed to contact LLW'1 regarding atter. dance at the v!arkshop.

Meeting with DOE to Discuss Cooperation On Site Remediation Efforts Staff from the Division of low-level Waste Management and Decommissioning (LLWfil met with representatives from the Department of Energy (00E) on May 14, 1991, to discuss site remediation efforts and issues.

This meeting was held to exchange iriformation about site remediation programs in our respective agencies.

The overall goal was to establish charnels of communication with DOE staff involved in specific issues of site remediation, particularly site remediation objectives and remediation technologies.

Issues discussed included:

the need for a common Federal approach to site restoration; the need for standards for site characterization and modelling; and sharing of information on the development of site remediation technologies.

DOE explained the objectives and status of its Integrated Demonstration Programs for site restoration, and the role of the Technology Integration Branch in DOE /EM. The DOE staff were highly receptive to increased contacts and more involvenent by the flutlear Regulatory Comission (f!RC) with their remediation programs.

Suggested follow-ups to the meeting include:

visits to DOE sites; NRC involvement (as observers, peer reviewers or participants) in DOE site remediation oversight committees; and continued interagency meetings both at technical and managerial levels.

Pathfinder Atomic Plant On fiay 14, 1991, the reactor pressure vessel (RPV) was successfully removed from the containment building of the old Pathfinder Atomic Plant, near Sioux Falls, South Dakota.

The RPV, which had previously been filled with pea gravel and concrete grout, was hoisted by crane through an opening cut in the containment building dome, and then lowered into a lay-down area outside.

The RPV package was 32 feet long, 11 feet in diameter, and weighed 245 tons.

It will be placed on a special 12-axle heavy-duty rail car for shipment by special train to the US Ecology low-level waste disposal site near Richland, Washington.

The special train will be limited to 35 miles per hour and will have to yield right-of-way to scheduled general freight.

The trip is schedule' for August and is expected to take about two weeks to complete.

NRC Staff Observation of U.S. Department of Eneray Surveillance s

During the week of May 6-10, 1991, the Division of High-level Waste Management (HLWM) quality assurance (QA) staff observed two internal surveillances of the U.S. Department of Energy (DOE)/ Office of Civilian Radioactive Waste Management (OCRWM) Technical and Management Support Services (T& MSS) contractor and Yucca Mountain Site Characterization Project Office (YMPO) QA programs in Las Vegas, Nevada. The objective of these surveillances was to evaluate the adequacy and implementation of the procedural controls necessary for preparation, review and approval of requirements for Exploratory Studies Facility design activities at YMP0 and T& MSS.

The-surveillance team identified some deficiencies in both of the QA programs surveilled.

However, these deficiencies are minor and should not have any significant impact on the quality of the prograns at T& MSS and YMPO.

The HLWM staff found the surveillances to be effective and that the procedural implementation for the activities observed is adequate.

1 Staff Observation of Yucca Mountain Site Characterization Project Office (YMP01 Surveillance of Sandia National Laboratories (SNL)

During the week of May 6-10, 1991, the Division of High-Level Waste Management (HLWM) staff observed a YMP0 surveillance of SNL in Albuquerque, New Mexico.

SNL is the U.S. Department of Energy YM00 contractor responsible for conceptual design and systems performance assessment of the repository. The surveillance focused on the training and qualification records of personnel working for SNL and the internal audit process. The HLWM staff found the surveillance to be satisfactory and performed in a professional manner.

Items of Interest Week Ending May 17, 1991 National Academy of Sciences Committee on Fracture Characterization and Fluid Flow On May 6-7, 1991, the initial meeting of the National Academy of Sciences' Committee on Fracture Characterization and Fluid Flow was held in Washington, DC.

The committee's principal objective is to prepare a study report which reviews, synthesizes and integrates recent research concerning techniques and approaches to fracture characterization and fluid flow in rock fractures.

The study is fun',ed in part by NRC's Office of Nuclear Regulatory Research and has a fifteen month duration. As requested by Dr. Jane C.S. Long, Committeo Chairman and Lawrence Berkeley Laboratory (LBL) Staff Scientist, Tom Nicholson, RES and NRC Liaison Representative provided a brief ovetview of NRC research issues and ongoing studies to address them.

Other liaisons and committee members also provided short overviews on nuclear waste, mining, tunneling, dams and underground reservoirs (i.e., oil, gas, water and geothermal) as they re-late to fracture characterization and fluid flow. Copies of the committee's prospectus, member-ship and agenda which were distributed are available from Tom Nicholson.

The committee prepared a dra't study outline and is beginning a literature review. The next meeting is scheduled for California July 23-25, 1991.

Accident Management NUREG/CR-5543, "A Systematic Process for Developing and Assessing Accident Management Plans," has been issued. The document describes an approach for developing criteria that would be used by NRC staff for assessing the adequacy of licensees' accident management programs.

This report is the first of two reports on this approach. A prototype process is described for developing an A/M program based on the five framework elements of an A/M plan. Based on this process.-a preliminary set of assessment criteria are derived. These preliminary criteria will be refined and improved when the remaining steps of the approach are completed, that is, after the prototype process is validated through application, which is currently in progress, and will form the basis for the second, and final reports.

Publication of "A Unified Interpretation of 1/5 to Full-Scale Thermal Mixing Experiments Related to Pressurized Thermal Shock," NUREG/CR-5677 This report is of work performed at UC Santa Barbara under contract to NRC.

Thermal mixing in relation to Pressurized Thermal Shock has been examined experimentally throughout the world in a variety of scales. These include the

-CREARE-1/5, the IV0/IVO(NRC)-2/5, the PURDUE(UCSB)-1/2, the CREARE-1/2, the HDR-1/1, and the UPTF-1/1 test facilities.

In this report the Regional Mixing Model and the associated computer programs REMIX and NEWMIX are used to interpret these data in a comprehensive fashion.

Office for Analysis and Evaluation of Operational Data items of Interest Week Ending May 17, 1991 Incident Response Branch (IRB)

On May 15. Headquarters participated in a full-scale emergency response exercise involving the Braidwood nuclear power plant, the State of Illinois, and Region Ill. Cornissioner Remick served as the Director of the Executive Team. All exercise objectives were addressed including implementation of the Federal Radiological Emergency Response Plan (FRERP). The Emergency Response Data System was effectively utilized during the exercise. A number of lessons were learned related to the interface with other Federal agencies.

In addition, since this was a safeguards drill, a Federal Bureau of Investigation (FBI) representative participated as provided for in the MOV between the NRC and the FBI.

Technical Training Center (TTC)

On May 13-14, the Training Advisory Group (TAG) met at the TTC. All regions plus NRR, NMSS, RES, OP, GPA, and AE00 were represented at the meeting.

A number of diverse training issues were discussed including the following:

the roles of the TAG and the Training Advisory Council; SECY-91-016. Technical Recruitment, Development, and Retention Planning and Implementation and its implications on the training and development of technical interns, personnel without prior nuclear experience, and personnel with prior nuclear experience; regulatory philosophy refresher training; post-course feedback questionnaires and questionnaires to survey the effectiveness of the overall development process; the Reactor Safety Course being developed by RES through Sandia National Laboratory; NRC inspectinn Manual Chapter 1245 discrepancies in certain sections; and program updates on a few TTC projects.

There was also a meeting of the Intern Program Coordinators at the same time as the TAG meeting, and both rneetings were consolidated for extensive discussions on program status.

Initial informatier, indicates a higher rate of hiring of reactor engineer interns and lower rate of health physics interns than projected.

On liay 14,-1991,:laria Lopez-Otin, Federal Liaison, attended the fir *t meeting of the SEAB Task Force on Civilian Radioactive Waste Management (Task Force).

The objective of the Task Force is to avlyze the institutional framework for managing radioactile waste and to sugge ; approaches for engendering in the public confidence so as to facilitate the Department of Energy's discharge of its statutory responsibilities in this area. Dur Mg the day-long meeting, the Task Force heard remarks from Secretary of Energy, James D. Watkins, and testimony from Dr. John Bartlett, 00E/0CWRM; Mr. Robert Bernero, NRC/NMSS; Dr.

D. Warner North, Member of the Nuclear Waste Technical Review Board; Mr. David Leroy, Nuclear Waste Negotiator; Mr. Loring Mills, EEI: anu Mr. Dan Reicher, NRDC. The Task Force plans to continue its fact-finding and deliberations and present its recommendations to the Secretary of Energy in a year.

CRCPD Annual Meeting The Conference of Radiation Control Program Directors, !cc. W.CPD) held its annual meeting on May 11-16, 1991 in Wichitas Kansas, ihe Radioactive Materials session included a discuss %n by Sheldon Schwartz, Deputy Director, GPA, on the status of NRC's compatibility review. Frederick Combs, Assistant Director, State, local and Indian Relations, spoke on NRC's pcrspective on low-level waste and Joel Lubenau, State Programs, spoke on radioactive materials in scrap metal.

A Workshop on the Art of Strategic Planning was conducted with John Montgomery, Region IV Deputy Administrator, serving as facilitator. Other workshops were held on Naturally Occurring and Artificially Produced Radioactive Meteritl (NORM & NARM), quality and efficacy of radiation control programs, exemptions from regulatory control, media communication, implementation of a licensing state program, emergency response planning, public education-and implementation of Clean Air Act provisions for radionuclide emissions.

A public meetino was held or May 14, 1991 for NRC to solicit inp-:t from the States on fCJr rd emaking proposals underway at the NRC.

Office of Enforcement Items of Interest Week Ending May 17, 1991 Significant Luforcement Action 1.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $75,000 will be issued on May 13, 1991 to Tennessee Valley Authority (Sequoyah). This action was based on the failure of control room personnel to respond to a control room clarm indicating low starting air pressure for the 2P-B emergency diesel generator, The low air _

pressure! condition continued for 1-2 hours until an auxiliary operator identified the condition locally at the diesel and reported the problem to the control room.

By EN_90-094 dated October 22, 1990, the Commircion was notified of tre intent of OE to issue a Notice of Violation and Proposed Impositioq of Civil Penalty in the amount of $5,000 to the Tripler Army Medical Center, Hawaii.

The action was based on the failure to comply with a procedure requiring licensee personnel to ensure, prior to the administration of radioactive substances, that the patient is not breast feeding an infant.

Because this failure-led to an incident in which a patient breast fed her three-week old infant after receiving a dose of iodine-131, which resulted in the destruction of the infant's thyroid function, it was classified at Severity Level I.

The licensee responded to the Notice in letters dated December 7 and December 21, 1990. After careful consideration of the licensee's responses and further consultation with the Commission, the staff mitigated the proposed civil penalty by 50% in recognition of the licensee's identification and reporting of the event and prompt corrective action. An Order Imposing Civil Penalty in the amount of $2,500 was issued May 13, 1991.

Civil-Penalty Paid Memorial Hospital of Laramie County, Cheyenne, Wyoming paid the civil penalty in the amount of $2,500. The action was based on a failure on the part of the hospital to ensure, through its Radiation Safety Officer, that all requirements in-10 CFR Part 35 were being met, as evidenced by 18 violations of-such requirements.

UFN selected the firm of DBI, Inc. of Washington, D.C. to provide space planning services for TWFN. DB1 will perform the space planning and interior design functions previously assigned to Dewberry & Davis.

It is now virtually certain that the FARE-SHARE subsidy, which has been provided by MCD01 to NRC White Flint employees who use public transit will be eliminated by the end of June.

The Director, Office of Consolidation, attended the MCDOT Sixth Annual Transpor-tation Awards Ceremony held at Strathmore Hall Arts Center on May 17.

Speakers at the breakfast meeting included Senator Paul Sarbanes, County Executive Neal Potter, County Council President Isiah Leggett, and MCDOT's newly appointed Director, Graham Norton. Senator Sarbanes reported that his colleague Senator Daniel Moyhihan had introduced a public transportation bill through the Finance Committee which includes a provision to raise the current IRS $15 cap on non-taxable transportation subsidies for employees to $60.

Mr. Potter noted in his remarks that it was inequitable that current laws permit valuable subsidies to single-occupant vehicle drivers in the form of free parking for employees but do not provide for public transit subsi6ies at comparable levels.

Emergency Preparedness Exercise at Haddam Neck l

On Saturday, May 11, 1991, the Connecticut Yankee Power Company conducted a partial-participation exercise at its Haddam Neck Plant. An NRC inspection team observed all activities and concluded that licensee performance was good.

This was a partial-participation exercise with voluntary participation by the State of Connecticut and 18 local towns.

FEMA did not observe any activities.

Tennessee Valley Authority On May 13, the Regional Administrator, accomoanied by the TVA Projects Chief, was at the Browns Ferry Nuclear Plant to hold a press conference at 10:00 a.m.

A TVA Management Meeting was held following the press briefing.

At 3:00 (CDT) the TVA Projects Chief was a guest on a live call-in program on Station WVNN, Athens, Alabama, 2.

Entergy, Inc.

On May 14, the Regional Administrator and selected staff members were at the Grand Gulf Nuclear Power Station to present the SALP Board results to representatives of Entergy Operations, Inc.

Georgia Power Company On May 16, the Regional Administrator and selected staff members were at Plant Hatch to present the SALP Board results to representatives of the Georgia Power Company.

Virginia Electric and Power Company - North Anna Exercise A small scale emergency exercise was conductedhon May 15, 1991, with Region II' staff observing for the NRC. The exercise was successful with one weakness identified for failure to adhere closely to emergency response procedures in the Technical Support Center.

Georgia Power Company - Vogtle Exercise A small scale emergency exercise was conducted with limited offsite participation on May 15, 1991, with Region II observing.

FEMA also had individuals on site to review selected positions of the medical response team. One violation was identified for an EOF HVAC which failed to perform its habitability function and one exercise weakness was identified for inadequate performance of a medical response team to a contaminated and injured worker.

Ireland Army Hospital An NRC Region II inspector was dispatched to Ireland Army Hospital in Fort Knox, Kentucky on May 17, 1991 to evaluate two diagnostic medical misadministrations. Although the misadministrations were in the diagnos-e tic range (less than or equal to 5 rems to whole body or any organ), this event is significant since the misadministration was caused by an untrained and unsupervised technician drawing up the nuclear medicine doses.

Chemetron Corporation On May 16, 1991, the Regional Administrator, Deputy Director, Division of Radiation Safety and Safeguards, and selected regional staff conducted an enforcement conference to discuss a violation associated with contamination of equipment and buildings in previously considered unrestricted areas of the Harvard Avenue site.

Braidwood Nuclear Power Station On May 15, 1991 Region 111 participated in an emergency preparedness (EP) exercise with the licensee. The regional effort consisted of "non pre-selected" base and site teams and deployment of the site team.

The licensee exercised their new EP procedures and the Region tested the newly revised incident Response Supplement to NUREG-0845 for site team members.

Commissioner Remick and Headquarters staff also participated in the exercise.

Kerr-McGee Chemical Corporation, West Chicago, Illinois On May 14, 1991, the State of Illinois and Kerr-McGee Chemical Corporation announced that thorium ore wastes at the site would be removed from the site and shipped to an undetermined, out-of-state location for disposal.

The agreement was part of a planned settlement of state and federal litigation involving Illinois, the City of West Chicago, and Kerr-McGee.

Details of the agreement remain to be determined.

Braidwood Nuclear Power Station On May 16, 1991 Region III dispatched two radiation specialists to the site to evaluate the circumstances surrounding extremely high radiation levels found on a reactor system 3 to 5 micron filter, and to evaluate the impact of a significant radiciodine primary coolant spike.

Perry Nuclear Power Plant On May 14, 1991, a routine management meeting was held at the Perry Nuclear Plant.

Participants included the the Director, Division of Reactor Projcets Ill/IV/V, Office of Nuclear Reactor Regulation (NRR); the Chief, Branch 3, Division of Reactor Projects, Region Ill; and the Executive Vice President, Centerior, and members of their staffs. The purpose of the meeting was to discuss the monthly performance report, forced outage work accomplished, Agastat relay replacement, and current operational events.

Fermi Nuclear Plant On May 10, 1991, a five person, two week regional team inspection of the licensee's modification implementation program was completed.

The licensee's_ design planning and field installation of modifications were good, and the licensee's post-modification testing program was acceptable.

However, weaknesses were identified in the documentation of in-progress and completed work,-and several examples of noncompliance with rcdiological protection procedures were noted.

Workshop to be Held in Boise, Idaho On Wednesday, May 15, 1991, Region IV conducted a workshop in Boise, Idaho, for NRC licensees located within the State of Idaho.

The workshop is intended to further inform the licensees about policies regarding the transition of regulatory authority from Idaho's Agreement State program to NRC, and to introduce the licensees to NRC's organization, regulations specific to materials programs, licensing and inspection policies for materials programs, and enforcement policy.

Conference of Radiation Control Program Directors, Inc.

The Region IV Deputy Regional Administrator spoke on the

" Art of Strategic Planning" at the 23rd Annual National Conference on Radiation Control on hay 13, 1991, and he attended program workshops on May 12 in Wichita, Kansas.

NRC Observation of Lift of Pathfinder Reactor Vessel A Region IV inspector and two NMSS representatives observed the lift of the Pathfinder reactor vessel out of the reactor building on May 14, 1991.

No radiological hazards are expected during this operation, a major step toward final deconmissioning of the Pathfinder reactor facility.

M,eeting with Chem Nuclear Systems, Inc.

The Director, Deputy Director, and senior stoff of the Uranium Recovery Field Office (URFO) met with a representative of Chem Nuclear Systems,-Inc. (CNSI) in Denver, Colorado. CNSI is the technical support contractor to DOE's Grand Junction Project Office, who is the lead-office for DOE on establishing procedures for assuming ownership of reclaimed and remediated Title I and Title II uranium mill tailings piles.

The purpose of this meeting was to discuss the status of reclamation of Title II sites licensed by NRC.

A briefing is scheduled in the near future on license renewal.

The briefing should include a discussion of the role of aging research in development of the rule.

The staff should address whether its approach to maintenance during a 40 year license period will provide an effective aging program that can be used to support license renewal, preliminary results of an evaluation of maintenance team inspection reports suggest that the current industry maintenance programs may not be effectively managing aging.

The staff should assess these results and address how this information is being factored into its recommendations on license renewal.

The staff should also discuss why the structures, systems and components which are important for maintenance are different from those important for license renewal.

The Commission requested the status of issuing aging related inspection guidance to NRC inspectors, and whether we have an effective means of enforcing findings in the aging area.

The Commission requested information on how the staff plans to assure that the results of the nuclear plant aging research are made known to the industry for consideration in plant ooeration and for license renewal.

It will take significant effort on the part of all the NRC offices to work together to see that results of this research effort are considered in our regulatory program for operating plants and license renewal.

Commissioner Curtiss requested information on staff plans to provide for at least one examination to confirm the location of the anti-vibration bars as a result of the Mihama event.

Commissioner Remick requested that the staff provide the Commission with a status report on the efforts of BWRs to replace the 304 stainless steel with 316 nuclear grade steel.

Commissioner Remick also requested that the staff provide the Commission annually a paper on the status of Research's programs on material aging.
