Gentlemen:

Subject: Combined Inspection 50-272/83-16, 50-311/83-15 and 50-311/83-18

This refers to the routine resident safety inspection and the special resident

safety inspection by Messrs. L. Norrholm and R. Summers on May 11 - June 8,

1983 and May 9 - 18, 1983, respectively, at the Salem Nuclear Generating

Station, Hancocks Bridge, New Jersey. The inspection consisted of document

reviews, interviews, and observation of activities. The findings were discussed

with Mr. J. M. Zupko, Jr. of your staff.

Based on the results of these inspections, it appears that certain of your

activities were not conducted in full compliance with NRC requirements. Two

of these violations are set forth in the Notice of Violation, enclosed herewith

as Appendix A. These violations have been categorized by severity level in

accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) published in

the Federal Register Notice (47 FR 9987) dated March 9, 1982. You are required

to respond to this letter and in preparing your response, you should follow the

instructions in Appendix A.

In addition to the two violations described in the attached Notice, two other

apparent violations involving failure to meet Technical Specification Limiting

,

Conditions for Operation for maintenance of a reactor coolant system vent path

and maintenance of vital AC electrical bus equipment, are detailed in the

attached reports. We are considering these items for appropriate enforcement

action and will be addressing them under separate correspondence at a later

time.

Based on our review thus far, there appears to be a breakdown in your control

over plant conditions, particularly as it applies to the interface between

operations and maintenance activities. We want to further discuss this issue

with you at an enforcement conference in our Region I office on July 22, 1983.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures

will be placed in the NRC Public Document Room unless you notify this office,

by telephone, with ten days of the date of this letter and submit written

application to withhold information contained therein within thirty days of

the date of this letter. Such application must be consistent with the require-

ments of 2.790(b)(1). The telephone notification of your intent to request

withholding, or any request for an extension of the 10 day period which you

believe necessary, should be made to the Supervisor, Files, Mail and Records,

USNRC Region I, at (215) 337-5223.
