ATTACHMENT TO LER# : 83-028/03L-o SUPPLEMENT TO CAUSE DESCRIPTION THE PORTION OF THE SURVEILLANCE TEST PERTAINING TO TD1 WAS IMMEDIATELY RETESTED 5 TIMES UNDER THE DIRECTION OF THE INSTRUMENT MAINTENANCE SUPERVISOR WITH NO PROBLEMS BEING FOUND. THIS POSSIBLE PROBLEM WITH TD1 WOULD NOT HAVE PREVENTED A SAFETY INJECTION (SI). HOWEVER, IT WOULD HAVE PREVENTED MANUALLY RESETTING THE SI, IF AN SI WAS INITIATED.

THE SSPS TRAIN A WAS DECLARED OPERABLE AND RETURNED TO SERVICE ON 3-31-83.

ON 4-21-83 THE SURVEILLANCE TEST WAS AGAIN PERFORMED SATISFACTORILY.

DURING THE UPCOMING REFUELING OUTAGE TD1 AND THE ASSOCIATED LATCHING RELAY (K-602) WILL BE INSPECTED. AN UPCATED LER WILL BE SUBMITTED IF ANYTHING UNUSUAL IS FOUND.
