APPENDIX A NOTICE OF VIOLATION Duke Power Company Docket No. 50-414 Catawba 2 License No. CPPR-117 As a result of the inspection conducted on June 26 - July 25,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violation was identified.
10 CFR 50, Appendix B, Criterion V, as implemented by Topical Report Duke 1-A, Section 17, Paragraph 17.1.1.5 requires that activities affecting quality be accomplished in accordance with prescribed drawings. Drawing flo.
CN-1051-40, requires that rebar spacing at the reactor coolant pump base blockouts be 1 1/16-inch minimum. Drawing Nos. CN-1051-47 and CN-1051-49 l
require that rebar spacing for the pressurizer shield wall be 11/16-inch minimum Contrary to the above, activities affecting quality were not accomplished in accordance with prescribed drawings in that:
1.
On flarch 26, 1981 rebar spacing at the reactor coolant pump base blockouts was zero to 3/4-inch in three places.
2.
On April 15, 1981 rebar spacing at the pressurizer shield wall was zero to one-inch in one area.
This is a Severity Level VI Violation (Supplement II.F.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
