NOTICE OF VIOLATION Virginia Electric and Power Company Docket No. 50-404 North Anna 3 License No. CPPR-114 As a result of the inspectieri conducted on July 27-31, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.
10 CFR 50, Appendix B, Criterion V, impl:mented by Appendix 3, paragraph A.1.3.6 of the PSAR, requires in part that, activities affecting quality-shall be prescribed by documented instructions, procedures and drawings and shall be accomplished in accordance with instructions, procedures' and drawings.
North Anna specification NAS 3048, " Specific Requirements for Equipment Storage - 011 (SRES-011)", states that Units 3 and 4 reactor containment dome plates shall be stored stacking individual plate assemblies one atop the other in separate stacks and that blocks will be placed between the plates to provide an air space between the adjacent plates.
Contrary to the above, the reactor containment dome closure plates did not have blocks placed tetween the plates to provide an air space between the adjacent plates as required.
This is a Severity Level VI Violation (Supplement II.F).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will' be achieved. Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amendea, this response shall be submitted under oath or affirmation.
