"A" ACCUMULATOR LOW LEVEL 1.

DESCRIPTION OF EVENT

With the unit at 100% power, operating personnel were starting to fill "A"'

accumulator tank when drain valve HCV-1852A was inadvertently opened instead of HCV-1851A (fill isolation valve).

"A" accumulator level dropped to'444 within seconds.

Immediate steps were taken to stop the drainage and refill-the accumulator. The "/" accumulater level was below the Technical Specifi-cations requirements outlined in 3.3.A.2 for a period of 15 minutes. This event is reportable in accordance with Technical. Specification 6.6.2.b.(2).

2.

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT SYSTEMS:

The redundant accumulators were available; therefore, the health and safety of the public were not affected.

3.

CAUSE

The event was caused by improper valve manipulati on: by operational personnel.

4.

IMMEDIATE CORRECTIV.E ACTION:

HCV-1852A was closed to prevent further decrease in level.

5.

SUBSEQUENT CORRECTIVE ACTION:

HCV-1851A was opened to restore tank level.

6.

ACTIONS TAKEN TO PREVENT RECURRE!;CE:

The operator involved was reinstructed-in the fill procedure for accumulat rs.

7.

CENERIC IMPLICATIONS:

None.
