dance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following. violation was identified.

10 CFR 50, Appendix B, Criterion XVI, " Corrective Action," states, in part, i
" Measures shall be established to assure that conditions adverse to quality, such as. failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected.
1 In the case of significant conditions adverse to quality, the measures shall assure that 'the cause of the condition is determined and corrective action.

The Oyster Creek Nuclear Generating Station Operational Quality Assurance Plan, Revision 5, dated April 1,1981,Section XI.G, states, in-part, "The -

establishment of suitable corrective action includes an evaluation of the i
nonconformance, giving consideration to its cause, similar conditions that may exist in other areas, its relationship to nuclear safety, and solutions that would prevent repetition of conditions adverse to quality..."
Contrary to the above, corrective actions were ineffective in correcting con-I ditions adverse to quality wh. ;h present the potential-for the uncontrolled i
release of radioactivity as evidenced by the following examples.
1.
In January 1980, an undetermined volume of water was released from pin-j hole leaks in underground condensate transfer piping.
2.
On May 16,1980, about 50 gallons of water was spilled from a level measuring device on the condensate storage tank.

On August 9,1980, about 500 gallons of water was released from a leaking air release valve on the condensate transfer system.

On December 4,1980, a pipe failure on a demineralized water transfer j
pump released an undetermined volume of water into the contaminated ccndensate transfer pump building.

On December 15, 1980, a pipe failure on a demineralized water transfer pump released about 7500 gallons of water into the contaminated con-densate transfer pump building.

01 March 14,1981, a packing failure on valve V-2-88 resulted in the release of about 50 gallons of water from the condensate transfer system.

On April 21, 1981, a packing failure on valve V-2-88 caused the release of a large volume of water (in excess of 10,000 gallons) from the condensate transfer system.
This is a Severity Level IV Violation (Supplement II.D.1) i Pursuant to the provisions of 10 CFR 2.201, Jersey Central Power and Light Com-pany is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged items of noncompliance; (2) the reasons for the items of noncompliance if admitted; (3) corrective steps which will be taken to avoid further items of noncompliance; and (4) the date when full compli-ance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirma-tion.
