BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - MAKEUP AND PURIFICATION AND DECAY HEAT REMOVAL PUMP MOTORS WITH DAMAGED THERMOCOUPLE WIRING - NCR'S 1087 AND 1101 - FINAL REPORT On November 21, 1979 M. J. Gouge, NRC-OIE Region II, was informed that Nonconformance (NCR) 1087 was determined to be reportable in accordance with 10 CFR 50.55(e). Since that time, related NCR 1101 has been determined to be reportable in accordance with 10 CFR 50 55(e). This was followed by our interim reports dated December 21, 1979, March 27, May 8, and October 7, 1980, and February 17, 1981. Enclosed is ouP final

report. This NCR was closed in inspection report 438, 439/81-15 dated June 4, 1981.

4 1 j Description of Deficiency

During an equipment inspection, the thermocouple wiring for the makeup i

and-purification and the decay heat removal pump motors had indications of overheating. While the damaged thermocouple wiring in itself does not represent a significant safety problem, the damage could be an indication of more extensive damage to the motors.

Safety Implications The testing conducted by B&W/ Westinghouse representatives onsite provided 4 no evidence of overheating of the motors. The damage to the thermocouples appears to be mechanical. Since the motors have been

tested and shown to be acceptable and the thermocouples perform no safety j- function, had the subject condition remained uncorrected, it could not
have adversely affected the safety of operations of the plant.

1 Corrective Action During the week of April 13, 1981 a representative of Westinghouse 1 visited the Bellefonte site. The purpose of his visit was to aid in the

, conduct of the testing of the subject thermocouple wiring. This i

representative verified that the testing was conducted in accordance with B&W Field Change Package (FCP) 110 (Repair and Testing of Decay Heat and Makeup Pump Motor Thermocouples).

The test procedure included an individual inspection of thermocouples.

The inspection provided no evidence of overheating. However, the thermocouple leads were mechanically dat:ged. The B&W representative witnessed the repairs dictated by FCP 110. After the final repair, the 4

motors were tested.

The motors are now serviceable and warranted tv B&W.
