Identification and Description of Occurrence 1.

On 7-21-S1, it was discovered that 1-FCV-30-56 and 1-FCV-30-57, containment purge isolation valves, had been closed at 0428 (c) on 7-20-81 and were not leak rate tested within the 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> as required by Surveillance Requirement 4.6.3.1.4.

purge isolation valves, had been closed at 1635 (c) on 7-29-81 and were not leak rate tested within the 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> as required by Sur eillance Requirement 4.6.3.1.4 Conditions Prior to Occurrence 1.

Unit 1 in Mode 3 with RCS temperature and pressure at 544*F and 2235 PSIC.

Unit 1 in Mode 1 at 100% Reactor Power.

Apparent Cause of Occurrence 1-HS-30-14 and L-1.

Mechanical test personnel were informed that handswitches HS-30-15 were operated and the purge valves needed testing. Test personnel performed leak rate tests on 1-FCV-30-14 and 1-FCV-30-15 but were not aware that the handswitches also operated valves 1-FCV-30-56 and 1-FCV-30-57.

These two valves were not tested.

Since valves 1-FCV-30-37 and 1-FCV-30-40 are frequently cycled, mechanical test 2.

Corrective Action Taken tested at i

1.

Valves 1-FCV-30-56 and 1-FCV-30-57 were satisfactorily leak rate 1800 (c) on 07-21-81.

Procedures are being revised to indicate the valves which are operated by each handswitch.

Valves 1-FCV-30-37 and 1-FCV-30-40 were satisfactorily leak rate tested at 1635 2.

(c) on 08-02-81.

Test personnel have been reinstructed concerning purge valves which are routinely cycled and when leak rate testing should be perfocmed.

Failure Data i

SQRO-50-327/80190 is the only previous occurrence of failure to leak test l

contcinment purge isolation valves within time requirements.
