P. O. BOX 361 PLATTEVILLE, COLORADO 80651 REPORT NO. 50-267/77-34(30)

Preliminary IDENTIFICATION OF OCCURRENCE:

On September 19, 1977, a number of errors were found in the seismic supports for portions of Class I, field run, two inch and under piping systems. This event has been identified as a reportable occurrence in accordance with Fort St. Vrain Technical Specification AC 7.5.2.b.3.

EVENT DESCRIPTIOM:

As the result of the review of seismic systems and equipcent that was per-formed following discussions with the Nuc1 car Regulatory coznission in May, l

1976, it was necessary to review the piping system support design for those portions of systems or equipment that were revised from C7 ass II to Class I s tatus.

This revision from Class II to Class I resulted in field checking l

appro:cimately 500 isometric drawings that defined the affected portions of

(

two inch and under piping systems in the plant.

These isometric drawings serve as the basis for Fort St. Vrain piping seismic certification.

t The re-f sults of 'he field check of these 500 isemetric drawings indicated that in addition t + the piping support revisions ' required because of Class I boundary l

extensions, other errors in the supports of the existing class I portions l

of the pipit.g syste=s also existed.

{

lowing eneral categories:

The errors identified fall into the fol-

1) }Hssing hangers
2) Improper hangers used in the installation
3) llangers not located in the proper location
4) System piping configuration does not agree with the isometric drawings CAUSE DESCRIPTION:

The cause of the errors in seismic support for some Class I, two inch and under piping systems has been identified as inadequate implementation of the Quality Assurance Program during the construction phase..
