Narrative

Report for LER 50-366/1981-071 On July 18,1981, at 0045, with the reactor at 63% power and while performing HNP-2-3111, Main Steam Isolation Valve Trip Test, the "D" MSIV, 2B21-F028D, failed to close.within the required time frame of 3 to 5 ' seconds as, per Tech Specs 3.6.1.

The closing time was 1.7 seconds.

There~was no effects-to the safety of the plant or public health due to this event.

The investigation of the failure revealed the hydraulic. fluid level of the valve operator. was low. A small piece of shim stock was found in the hy-draulic seal area which could have. attributed to the failure.

The shim stock was removed and.the hydraulic seal inspected and'.no damaged to the seal was found.

The operator was charged with nitrogen and the hydraulic fluid level brought back to normal.

The valve was retested satisfactorily and returned to service.

The generic review revealed no inherent problems.

The failure due to low hydraulic fluid level is repetitive as last reported on Reportable Occurrence Report No. 50-366/1981-045.. The unit is now.in full compliance and no further reporting is required..
