Description of Event

On July 2, 1981, during Mode 3 condition, the control rcom operator noticed that the TAVE Protection Channel Indicator (TE-24320) failed high. The Protection Channel was placed in the trip mode within I hour. This item is contrary to T.S. 3.3.2 and reportable pursuant to T.S. 6.9.1.9.b.

Probable Consequences of Occurrence The consequences of this event were limited because the affected channel was placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and the remaining redundant channels were available and functioning properly.

As a result, the health and safety of the general public were not affected.

Cause of Event

The cause of this event is not known at this time. However, the indicator read full scale with RCS temperature at 463 F which is an indicator of an "open" failure.

Immediate Corrective Action

The failed RTD was placed in the trip mode and an installed spare (TE-2432C) was placed into service. A new temperature sensor was installed in TE-2432D. The new RTD was calibrated and placed back in service.

Scheduled Corrective Action No scheduled corrective action is taken.

Actions Taken to Prevent Recurrence No action is taken to prevent recurrence.
