Idc n t ification and l'c:.t:riptjyn of ( 3:curretjee 3-A reactor recirculation pump tripped due to M/G set tripping Conditions Prior to Occurrence:

Unit 1 - refueling outage Unit 2 - modification outage.

Unit 3 - at 98%.

A ct ion specified in the Technical Specification Surveillance Requirements met due to inoperable ettuinment.

Describe.

Restored 3-A recirculation pump to service within 1 hours1.157407e-5 days
2.777778e-4 hours
1.653439e-6 weeks
3.805e-7 months
.

Apparent Cause of Occurrence:

The reactor recirculation M/G set tripped due to 2A-K45A relay action initiated by RTD high temperature trip.

Au:dvsis of Occurrence There was no danger to the health or safety of the public, no release of activity, no damage to the plant or equipment, and no resulting significant chain of events.

Corrective Action

A seecial test is being written and will be imolemented to investigate this ptoblem. Recurrence control will depend on results from these investigations. By 6/27/81, the trip setting for the drive motor high temp and Ren hi temp relays for each M/n set on all three units was raised to trip at 120 centigrade. Alarm annunciates at 110 centigrade, Failure Data:
