CONTROL TRANSFORMER FOR FUEL OIL PUMP (EE-P-lC) FAILED 1.

DESCRIFrION OF IVENT:

With Unit No. 2 at 100% power, the operator attempted to start one of the two fuel oil pumps serving the No.3 emergency diesel generator, while taking readings for the daily operator log. The control transformer serving the fuel oil pump failed and burned. This resulted in the loss of fuel oil pump EE-P-lC and therefore, the loss of one of two fuel oil flow paths to the No.

3 EDG. The event is contrary to T.S. 3.16.A.5 and is reportable per T.S.

PROBABLECONSEQUENCESANDjSTATUSOFREDUNDANTEVENT:

The redundant fuel oil flow path to the No. 3 EDG was verified operable by PT-22.2; therefore, the health and safety of the public were not affected.

CAUSE OF EVENT

A random failure of the control transformer for the EDG fuel oil pump motor rendered the flow path inoperable.

4.

IMEDIATE CORRECTIVE ACTION:

i The immediate corrective action was. to verify the operability of the redundant flow path and notify the electrical department to make the necessary repairs.

5.

SUBSEQUENT CORRECTIVE ACTION:

i Upon completion of the maintenance work and verification that both fuel oil flow paths were operational, no subseq. tent corrective action was required.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

The failure of the transformer was deemed a random failure; therefore, no further action is required.
