Y. Crlenjak on May 11, 1981 in accordance with 10 CFR 50.55(e) as NCR 1449. 'nolosed is our first interim report. We expect to submit cur next report by August 27, 1981. We consider 10 CFR Part 21 applicable to this deficiency.

FIRST INTERIM REPORT Description of Deficiency The insulation ' surrounding the reactor vessel is supported by a steel framework supn11ed by Mirror Insulation. While installing the reactor vessel bottu._

d insulation supoort steel, it was discovered that practically all of the weld preps cabcns for field welding and vendor-made welds on these structures were faulty. The weld preparations were either not prepared in accordance with the vendor's drawings or omitted. The vendor welds do not meet the visual acceptance criteria of the American Welding Society (AWS) D1.1-77 structural steel code. The defects include spatter, are strikes, undercut welds, craters, porosity, and undersize or incomplete welds.

These deficiencies should have been discovered before Babcock and Wilcox (B&W) released the structures for shipment from its subcontractor's (Mirror Insulation) plant.

B&W also failed to provide TVA's inspector the opportunity to perform source inspection as required by the contract.

Interim Progress TVA has notified B&W of this deficiency and is working toward a resolution.

There have been no similar deficiencies in the past for insulation support steel for the reactor vessel and other reactor coolant system components.

An investigation is being made to determine if similar deficiencies exist for insulation support steel supplied by Mirror Insulation for other Bellefonte components or for other TVA plants.
