NRC BULLETIN 88-02 ON STEAM GENERATOR TUBES (MPA X802)

(TAC NOS R00328. R00329, 67323 AND 67324) - SEQUOYAH NUCLEAR PLANT, UNITS 1 AND 2 By letters dated March 1. March 31, July 6 and August 2, 1988; July 5, 1989; and January 24, 1990; you have responded to NRC Bulletin 88-02, " Rapidly

-Propagating Fatique Cracks in Steam Generator Tubes." This bulletin was issued on February 5,1988 and requested you to implement actions specified in the bulletin to minimize the potential at Sequoyah for a steam generator tube rupture caused by a rapidly propagating fatique crack, such as occurred at North Anna Unit 1 on July 15, 1987.

Sequoyah is a plant with Westinghouse steam generators having carbon steel support plates. The March 1 and-August 2,1988 letters provided the basis for the restart of Unit 2 and Unit 1, respectively, in 1988, from their extended outage since 1985.

The staff's evaluation of the responses to the bulletin are contained within-the enclosed Safety Evaluation. We conclude that the actions taken at Sequoyah fully resolve the issues in the bulletin and are acceptable. This conclusion is based on you adopting administrative controls at Sequoyah to ensure that significant changes to the operating parameters of the steam generators are reviewed for changes to the bulletin analyses for the units.-

.This has been discussed with your staff and their decision was to add sufficient information in the Sequoyah Final Safety Analysis Report (FSAR) so that such changes to the steam generators would be identified through the Sequoyah 10 CFR ta%a 50.59 program and properly reviewed.
