10 CFR 50, Appendix B, Criterion XV specifies that measures shall be established to' control materials, parts, or components which do not con-form to requirements in order-to prevent their inadvertent use or instal-lation.

It further. specifies that nonconforming items shall be reviewed and accepted, rejected, repaired or reworked in accordance with documented procedures. ' Procedure AP-0207, Rev. -8, " Equipment Control," specifies in i

step II.A.7.c.,_ that in cases where required documentary evidence is not -

3 available, the associated equipment or materials shall be considered non-1 conforming.

Procedure AP-0212, Rev. 16. " Control of Purchased Material, Equipment and Services," requires in step II.A.4., that documentary evi-dence that items conform to procurement requirements shall be available prior to installation or use of such items where required by code, regu-lation or contract requirements. Procedure AP-0206, Rev. 10M, "Noncon-formance: Report," requires in discussion step No. 4, nonconforming mate-i rials, that parts or components which depart from specified requirements that_ render the quality of an item. unacceptable-shall be reported under.a i

Nonconformance Report (NCR).

Procedure Ap-0206 further specifies in step A.lli that the Licensee may authorize the materials or servicas in ques-U tion to be placed in service, on a permanent or temporary basis, after the

~l NCR-is approved.

LContrary to the above on November 8 and 9, 1989, the licensee installed nonconforming, commercial grade A194-2H fasteners (hexagonal nuts) on the 4

No. 2 and No. 3 charging pump strainers (Safety Class 2) without-the re-quired documentary evidence of conformance.

Further, the use of noncon-

-forming materials was allowed to occur without the required approved NCR.

This is a Severity Level IV (Supplement I).

Pursuant to 10 CFR-2.201 Yankee Atomic Electric Company is hereby required to submit to this office within thirty days of the date of the letter which trans-mitted-this notice, a written statement of explanation in reply, including:

(1) the corrective steps which will be taken to avoid further violations; and (2)'the date when' full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.

The response directed by this Notice is not subject to clearance by the Office of Management and Budget under the Paperwork Reduction Act of 1980, PL 96-511.
