Combined Inspection Report Nos. 50-334/89-22 and 50-412/89-21

This letter refers to the routine resident safety inspection by Messrs.

J. Beall and P. Wilson at the Beaver Valley Power Station in Shippingport,

-

Pennsylvania. The reporting period covers the time from October 14, to

November 17, 1989. The inspection consisted of document reviews, interviews

and observation of activities. The findings were summarized and discussed with

you and members of your staff at the conclusion of the inspection.

Based on the results of this inspection, it appears that in one instance,

activities were not conducted in compliance with NRC requirements. The failure

to accurately-follow procedures led to the loss of one train of the Cold

Overpressure-Protective System.

In addition, other weaknesses were identified

in the control of licensed activities which contributed to the above event.

1The response directed by this letter and the accompanying Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1989, PL 96-511.
