-w APPENDIX A Notice of Violation Duquesne Light Company Docket Nos. 50-334 and 50-412 Beaver Valley Power Station, License Nos. DPR-66 and NPF-73 Units 1 and 2 As a result of the inspection conducted on October 14 - November 17, 1989, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the following violation was identified:
Technical Specification 6.8 requires that written procedures be established, implemented and maintained covering activities listed in Appendix A of Regulatory Guide 1.33, Revision 2. February 1978.
Site Administrative Procedure 41, Clearance Procedure Section VI. C. 6. d.
8 requires that the applicable control room status board print be updated following the posting of a clearance.
Contrary to the above, on November 9, 1989, the applicable control room status board print was not updated following the posting of clearance No.
562735.
This led to the loss of one required train of the Cold Overpressure Protection' System.
This is a Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is
-hereby required to submit to this office within thirty days of the date of
'the letter which transmitted this Notice, a written statement or explana-tion in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to i
