5 N/A JK SC Lead reduced when Operators intention-ally tripped a Reactor Recirculation pump to recover level from loss of a feed pump. The feed pump turbine control valve pneumatic positioner l
orifice was blocked by a loose metal chip cansing t.losure of the valve on loss of motive air and an ensuing decrease in turbine speed without a pump trip. The metal chip was removed during inspection.

3 89-026 LD FLT,TS An automatic reactor scram occurred due to MSIV drift and subsequent closure trip signal when an instrument air dryer post-filter element caught fire and the post-filter vessel failed, resulting in loss of instru-ment air.
The dryer temperature limit switch was determined to be faulty during instrument air system troubleshooting and testing.
