O APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-553/80-18 and 50-554/80-16 Phipps Bend 1, 2 License Nos. CPPR-162 and CPPR-163 As a result of the inspection conducted on December 17-19, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified.
A.
10 CFR 50, Appendix B Criterion V requires that activities affecting quality shall be prescribed and accomplished in accordance with procedures. TVA procedure QCI C-207 and drawing number 4KE 0401-5K-11R requires that vertical reinforcing bars in concrets pour number KAC01-34, 37, 40, 43, be placed on 12 inch centers with a plus or minus tolerance of 2 inches.
Contrary to the above, in concrete pou: number KAC01-34, 37, 40, 43 the vertical reinforcing steel was not placed in accordance with procedure and drawing requirements.
This is a Severity Level V Violation (Supplement II.E.).
Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit to this cffice within twenty-five days of the date of this Notice, a written 7.tatement or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have-been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and. (5) the -
date when full compliance will be achieved. Under the authority of Section 182 of the Atcaic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
