Reference:

1). Letter, G.S. Spencer to D.E. Dobson, NRC Inspection at WNP-3/5, dated February 12, 1981.

This letter is in response to your letter of February 12,1981, which discussed the results of the inspection conducted January 13.thru 16, 1981, of activities authorized by Nuclear Regulatory Commission Construc-tion Permit Numbers CPPR-154 and CPPR-155. The letter identified one (1) item of noncompliance and required the Supply System to provide a response to these items.

The specific Nuclear Regulatory Commission Finding, as stated in your t letter, and the Supply System response is provided in Attachment I of this letter.

~ ATTACHMENT I NUCLEAR REGULATORY COMMISSION FINDING:

As a result of the inspection conducted on January 13 to 16,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),

the following violation was identified:

10 CFR 50, Appendix B, Criterion V, as implemented by Section 17.4.5 of the Quality Assurance Program documented in the approved PSAR deviation 18-WP, states, in part, that " Activities affecting quality. ..shall be occomplished in accordance with... instructions, procedures or drawings."

Morrison-Knudsen Administrative Instruction Procedure Number 15,

" Stud Welding Inspection Procedure," states in paragraph 6.3 that, "Each operator shall bend the first two studs on each day's production to 300 and the first two studs on each member to 30 0", in paragraph 6.8 that, "The operator shall circle with a paint stick, or paint a line up the shank of the first two studs bent on each new mea,ber", and in paragraph 6.5 that, "All ferrules must be broken off by the operator prior to inspection".

Contrary to the above, on. January 14, 1981 on the southside of Un;t 5 elevation 362', the inspectors observed the following:

(1) A stui welding operator welding on a member between columns Nos. CA-12 and CA-13 had welded approximately sixty studs without bending the first two studs.

(2) A number of studs were found which were bent, but had not received 'the necessary paint mark. It was later discovered that the Quality Control Inspector had been marking the studs, not the operator.

(3) Numerous studs were found with parts of the ferrules still in pl ac e. These studs had been inspected and accepted by the Morrison-Knudsen Quality Control Inspector.

This is a Severity Level V violation.

Corrective Steps Which Have Been Taken And The Results Achieved.

A training session on Morrison-Knudsen procedure AI-15, revision 2 was conducted on January 15, 1981. This session emphasized the requirements for removal of ferrules and the operator marking of the first two studs bent. Applicable Quality Control inspectors and construction personnel were in attendance. Morrison-Knudsen construction was directed to remove all ferrule material in placement ABS 017 362.5 prior to placing rebar.

The removal of this material will be documented on Morrison-Knudsen form QC-19. -

Construction personnel have been issued direction concerning the marking of studs bent by the operator.

Corrective Steps Which Will Be Taken To Avoid Further Items of Noncompliance:

The additional training conducted on January 15, 1981, supplemented with
