Report No. LER 81-016/03L-0

Description of Event

On February 5,1981, while operating in Mode 1 at 100% power the temperature indicator (TI412D), associated with T-AVG protection for Loop A, was reading erratically and approximately 5*F-15*F low after being placed back in service upon completion of a periodic test on that protection channel. This event is reportable pursuant to T.S. 6.9.1.9.b.

Probable Consequences of Occuregnce Since two redundant protection channels remained available to initiate the appropriate safeguarda and reactor protection actuation if required, and the affected channel was placed in trip within one hour, the health and safety of the public were not affected.

Cause of Event

The affected T-AVG protection channel was reading low and erratically due to a failed relay card within the circuit.

Immediate Corrective Action

The immediate corrective action was to place the T-AVG protect 4.on channel in a trip condition. After finding the problem to be a fa'ted relay card, the card was replaced, calibrated and the loop returned to service in accordance with the appropriate calibration procedure.

Scheduled Corrective Action No further corrective action scheduled.

i Actions Taken to Prevent Recurrence No further action needed.

Generic Implications There are no generic implications to this event.
