i ypggnt Cause of Occurrence:

A screw came loose from a terminal block and this interrupted power to both smoke detectors in a fixed spray zone. Actual cause of screw coming loose is unknown. Loose screw found 12/17/80. Analysis of event causing smoke detectors to be inoperable was made on 1/6/81.

Analpis_ cf Occurrence:

There was no damage to plant equipment. There was no activity release, no personnel exposure or injury and to danger to the health or safety of the

Corrective Action

The screw was replaced in the block and the other terminal blocks in the panel were checked for lease screws. No,other loose scress were found.
