Description of Event

On December 18, 1980, with the Unit in Mode 1, water was found leaking from a weld joint that connects vent valve 2-CH-185 to piping that tees into the suction line of charging pump 2-CH-P-1B.

Probcble Consequences of Occurrence Upon discovery of the leak the piping was isolated and charging pump 2-CH-P-1B was declared inoperable. Since there were two charging pumps operable the health and safety of the general public were not affected.

Cause of Event

The actual cause of a crack developing near the center of the weld material is unknown at this time. This crack may have been caused by improper fit-up of pipe to the socket weld fitting and by vibration from the charging pump.

Immediate Corrective Action

The leak was immediately isolated, charging pump 2-CH-P-1B was declared inoperable and an Emergency Maintenance Report was submitted.

i The weld was repaired and liquid penetrant inspected by approved welding procedures.

Scheduled Corrective Action None scheduled at this time.

Actions Taken to Prevent Recurrence None scheduled at this time.

Generic Implications We have experienced several failures of this kind recently and are investigating the cause of these welding failures.
