APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket Nos. 50-277 50-278 Based on the results of an NRC inspection conducted on August 1-31,1980, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC License Nos. DPR-44 and DPR-56 as indicated below.
Item A is an infraction, Items B and C are deficiencies.
A.
Technical Specification 6.8.1 requires in part:
" Written procedures and.
administrative policies shall be established, implemented and maintained...".
Administrative Procedure A-30, " Plant Housekeeping Controls"; Revision 1, dated January 14, 1980 requires in part:
" Cleanliness. The work areas shall be kept sufficiently clean and orderly so that plant activities can proceed in an efficient manner.
Garbage, trash, scrap, litter, and other excess materials shall be collected, removed from the work site, or disposed of in accordance with procedures or standard practices...".
Contrary to the above, the Unit 2 drywell was not properly cleaned for resumption of operation following refueling outage; in that, after shift supervison had inspected the drywell on August 5, 1980 and loggcd satisfactory results, the inspector found a large piece of scaffolding material, a scrap piece of deck grating, a scrap piece of I-beam steel, scrap hanger material, numerous pieces of loose wire, welding rods, and other debris in the drywell on August 6, 1980.
B.
Technical Specification 6.9.2.b
" Thirty Day Written Reports", states in part, "The reportable occurrences listed below shall be the subject of written reports...within thirty days... Reactor protective system or engineered safety feature instrument settings...found to be less conservative than those established by the technical specifications...".
Contrary to the above, on May 16, 1980, a pressure switch providing an input to a Unit 3 engineering safety feature, shutdown cooling high pressure isolation, was found to have its setpoint above the Technical Specification allowable range; and LER number 3-80-11/3L detailing this occurrence was not provided to the NRC until August 14, 1980.

fast closure was found to have its setpoint out of the Technical Specifica-tion allowable range, and LER number 3-80-12/3L detailing this occurrence was not provided to the NRC until August 14, 1980.

C.
Environmental Technical Specifications, Section 3.0, " Monitoring Requirements",
states in part, "When less than three cooling towers are in operation and the ficw...through the Conowingo Pond...is less than 15,000 cfs, thermal plume mapping shall be performed...The results...shall be submitted to the NRC within 30 days...".
Contrary to the above, results of required mapping conducted during the weeks of June 25, July 1, and July 8,1980 were not transmitted to the NRC until August 14, 1980.
