I.
Narrative Summary of Operatina Exoeriences Limerick Unit 1 began the month of December at a nominal 78%
of rated thermal power.
Reactor power had been reduced to 73%
on November 30 to support a control rod pattern adjustment.
Reactor power was restored to 100% on December 1.
Two other power reductions from 100% were performed during the month to r
L support control rod pattern adjustments.
Power was reduced to 85% on December 15 and power was reduced to 90% on December 29.
Limerick Unit 1 ended the month of December in OPCON 1 at a nominal 100% of rated thermal power.
Operational events that occurred during the month of December included:
On December 13, a four hour notification was made to the NRC after plant personnel discovered several failed spot welds on the' "A"
Standby Gas. Treatment System (SGTS) filter train.-
These failed spot welds created holes between the outer down stream screen and the filter mounting frame which allowed charcoal to leak out from the filter bed. The displacement of the charcoal created a possible bypass leakage path through the filter bed which could have prevented the fulfillment of the safety function of the "A"
train of SGTS.
On December 14, a boroscope was used to perform a visual inspection of the area under the "B"
SGTS filter.
The inspection was inconclusive due to the inability to access the total area under the filter bed.
On December 18, the "B" SGTS filter was dismantled and plant personnel found that a small amount of charcoal had leaked from the "B" filter.
However, the filter bed was still filled above the top of the screen indicating the amount f charcoal leakage was not enough to cause a bypass leakage path.
Both filter trains have been repaired, tested and returned to service.
II.
Challences to Main Steam Safety Relief Valves There were no challenges to the Main Steam Relief Valves during the month of December.

Narrative Summary of Ooeratina Exoeriences Limerick Unit 2 began the month of December in power operation at a nominal 100% of rated thermal power.
On December 1, the Unit 2 reactor was scrammed as part of the Main Steam r
Isolation Valve (HSIV) closure startup test and Hot Shutdown was entered.
The reactor was subsequently deprensurized and Cold Shutdown was entered on December 2.
This shutdown began
-a scheduled mini-outage to complete various maintenance l
activities and testing.
Upon completion, Unit 2 entered Startup on December 22.
Power Operation was entered on l-December 24 and reactor power was increased to 80% by December L
26.
On December 27, reactor power was reduced to 50% to I
support a scheduled main condenser tube leak inspection on the "2B" waterbox.
Following the inspection, reactor power was restored to 100% on December 30.
On December 31, reactor power was reduced to 87% to support backwash and precoat operations on the condensate filter demineralizers.
Limerick Unit 2 ended the month of December in power operation at a
nominal 87% of rated thermal power.
Operational events that occurred during the month of December included:
On December 1, the Unit 2 MSIV closure startup test was performed causing a planned reactor _ scram.
During the event, Reactor Pressure Vessel (RPV) water level reached
-43 inches and RPV pressure peaked at 1135 psig.
High Pressure Coolant Injection (HPCI) and Reactor Core Isolation Coolant (RCIC) initiated automatically and were used for RPV level and pressure control.
The Safety Relief Valves also lifted to control RPV pressure. After the unit was stabilized the MSIV's were opened and the RPV was depressurized and placed in cold shutdown using the main turbine bypass valves.
On December 25, while increasing Unit 2 generator load, a loss of stator coolant generator runback occurred when generator current reached 7469 amps.
The runback was caused by low stator cooling pressure due to a reduced generator hydrogen pressure.
Hydrogen pressure was increased from 35 psig to 50 psig, stator cooling pressure was restored, and load set was then raised to close the four turbine bypass valves which had opened during the runback.

II.
Challences to Main Steam Safety Relief Valves On December i during'the MSIV closure'startup test, several Unit 2' Main. Steam Relief ~ Valves (MSRV's) were ' challenged.
During the initial pressure' increase, the ' 2D', ' 2H ', and ' 2S '
MSRV's lifted per design reducing reactor pressure. After the three MSRV's closed, reactor pressure increased again and the
'2B',
'2D',
'2F',
'2H',
'2K',
'2M',
'2N' and 82S' MSRV's lifted.
The eight MSRV's closed per design following a reduction 'in reactor pressure.
A _ subsequent increase in reactor pressure was terminated when the.
'2K' MSRV was intentionally opened manually by the. control' room operator.

Following closure of the
'2K' MSRV, reactor pressure was controlled using HPCI and RCIC.
