Description of Event

OnSeptember9,Ig80itwasnotedthatthesteamflowindicatorFI 2494 was reading.7x10 pounds / hour with little or no actual steam flow.

The channel was placed in the trip mode within one hour as per TS.

3.3.2.1.

't Probable Consequences of Occurrence The channel FI 2494 was placed in the trip mode and the two redundant channels remained operable at all times. Therefore, the health and safety of the public were not affected.

Cause of Event

The exact cause of this event is not known at this time. The calibration checks are correct and are consistent with the other channel settings. However, in the range between 0% and 10% steam flow, the instrument response is non-linear. When above 10% steam flow, this response becomes linear.

Immediate Corrective Action

When the power (steam flow) increased to above 10%, the channel indicated properly. Therefore no immediate corrective action was necessa ry.

Scheduled Corrective Action The instrument will be examined during the next scheduled Mode 5 outage in order to identify the cause of this problem. Corrective action will be taken as required.

Actions Taken to Prevent Reccurrence Actions required to prevent recurrence will be determined when the exact cause is known.
