O APPENDIX B NOTICE OF VIOLATION Alabama Power Company License Nos. NPF-2 Farley Facility NPF-8 As a result of the inspection conducted on December 1-12, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.
Technical Specification 6.8.1 requires that written procedures be estab-11shed, implemented and maintained.
Contrary to the eb - written procedures were not established, implemented and maintained 'n tn.c:
a.
On November 18, 1080, a maintenance work request for work on a residual heat removal system check valve was released for work although the plant conc'itions necessary for performing this work were not estab-lished as required by plant procedure FNP-1-AP-52, paragraph 6.7.
The valve was not isolated.
b.
On August 5, 1980, a maintenance work request for removal of a steam generator blowdown pump ~ in Unit I was released for work although the plant conditions necessary for performing the work were not established as required by plant procedure FNP-1-AP-52, Paragraph 6.7.
The drain line valve was not properly set.
c.
On June 12, 1980, a maintenance work request was released for work altnough the plant conditions necessary for performing the work were not established as required by plant procedure FNP-1-AP-52, paragraph 6.7.
The IB waste gas compressor unloader manual isolation valve was not closed.
d.
On June 6, 1980, the tagging official autnorized removal of tags returning the reactor coolant systems and pressurizer liquid and steam a
sample lines at the primary sample sink without completing the "posi-tioning ' action" portion of the tagging operations order as required by plant procedure FNP-0-AP-14, paragraph 6.1.2.
Improper valve position and tag removal sequence resulted in a spill of radioactive liquid at the primary sample sink.
This is a Severity Level V Violation (Supplement I.E).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within twenty-five days of the date of this Notice, a written state-ment or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective 8105190 g
-steps which have been taken and the results achieved; (4) corrective steps which will be taken.to avoid further violations; and (5) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
