Dear Mr. O'Reilly:
This letter responds to the violation in the subject inspection report which states:
Technical Specification 6.8.1 requires that written procedures be established, implemented and maintained.
Contrary to the above, written procedures were not established, im-p!emented and maintained in that:
a.
On November 18, 1980, a maintenance work request for work on a residual heat removal system check valve was released for work although the plant conditions necessary for per-forming this work were not established as required by plant procedure FNP-1-AP-52, paragraph 6.7.
The valve was not isolated.
b.
On August 5,1980, a maintenance work request for removal of a steam generator blowdown pump in Unit 1 was released for work although the plant conditions necessary for performing the work were not established as required by plant procedure FNP-1-AP-52, paragraph 6.7.
The drain line valve was not properly set.
c.
On June 12, 1980, a maintenance work request was released for work although the plant conditions necessary for performing the work were not established as required by plant procedure l
FNP-1-AP-52, paragraph 6.7.
The 1B waste gas compressor un-l loader manual isolation valve was not closed.

On June 6, 1980, the tagging official authorized removal of tags returning the reactor coolant systems and pressurizer liquid and steam sample lines at the primary sample sink with-out completing the " positioning action" portion of the tagging operations order as required by plant procedure FNP-0-AP-14, paragraph 6.1.2.
Improper valve position and tag removal sequence resulted in a spill of radioactive liquid at the primary sample sink.
Discussioq While the incidents cited in the above violation did occur, we submit that the violation, as written, does not recognize the incidents being self-identified and investigated and documented in radiation incident re-ports for three of the incidents and in the operations night orders for the other.
Nor did it reflect the use of radiation incider.t reports as an instructional aid for discussion with the appropriate personnel.
Reason for Violation Personnel error, i.e., inadequate attention to detail by the responsible Shift Supervisor and/or the Shift Foreman inspecting.
Corrective Steps Which Will Be Taken To Avoid Further Violations The aggregate of in idents will be used to re-emphasize the necessity for strict adherence to procedures by:
1.
discussing with the individuals in each case to emphasize the need for attention to detail and careful planning.
2.
using the Radiation Incident Reports during retraining to emphasize the need for increased care and diligence in releasing equipment for maintenance and insuring that the people working on the systems understand the plant cond'tions.
3.
re-emphasizing by an operations memorandum to all Shift Supervisors ans Shift Foremen their role in the control of equipment.
