This refers to the routine inspection conducted by Mr. I. T. Yin of this

office on December 2-3, 1980, of activities at the William H. Zimmer Power

Station authorized by NRC Construction ?ermit No. CPPR-88 and to the dis-

cussion of our findings with you in a meeting held in your office on

December 16, 1980.

The enclosed copy of our inspection report identifies areas examined during

the inspection.

Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

As a result of our inspection, you issued Stop Work Orders discontinuing all

activities of Reactor Controls, Inc. (RCI), stopping the voiding of

Henry J. Kaiser (HJK) nonconformance reports, and stopping the preparation

of Design Document Control records on all pipe supports. We confirmed the

stop work on RCI activities with an Immediate Action Letter dated

December 24, 1980, and required NRC review prior to lifting of the stop work.

During this inspection, certain of your activities appeared to be in non-

compliance with NRC requirements, as described in the enclosed Appendix A,

and a written response is required.

We are concerned about Cincinnati Gas and Electric Compan.'s quality

assurance program regarding the activities of RCI and in general, large

and small bore piping activities, as demonstrated by repetitive noncom-

pliance in this area during the past three years.

In respondicg to this

letter, you are requested to address, in addition to your reply to the

specific violations, the management actions you have taken, or plan to

take, to improve the effectiveness of quality assurance program implementa-

tion with regard to small and large bore piping activities.

We held an enforcement conference with you on January 28, 1981, to discuss

our concerns. As stated during that meeting, the identification of additional

<

violations in future inspections which demonstrate ineffective management

actions in this area, will likely lead to escalated enforcement action. Also,

as you know, we are currently investigating other aspects of the QA program

at Zimmer.

I plan to meet with you again to discuss the findings of this

investigation.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter, the enclosures,

and your response to this letter will be placed in the NRC's Public Document

Room, except as follows.

If the enclosures contain information that you or

your contractors believe to-be proprietary, you must apply in writing to

this office, within twenty-five days of the date of this letter, to withhold

such information from public disclosure. The application must include a full

statement of the reasons for which the information is considered proprietary,

- and should be prepared so that proprietary information identified in the

application is contained in an enclosure to the application.

We will gladly discuss any questions you have concerning this inspection.
