3 81-022-03L-0 HA VALVEX The "Moog Valve" in the electro-hydraulic control system failed causin j the turbine stop valve to open Wily, simultaneously with 2 turbine governor valves. This caused a depressurizatio1 i

of "A" steam generator which resulted in a Main Steam Rupture Matrix actua-tion.

Ultimately, the result was an anticipatory reactor trip on low steam generator level. The unit remined down to repair a main steam icolation valve damaged in the transient and to investigate why RCV-14 failed to open.

The outage was further extended by a CRD stator replacement and the discovel y of water in the lower oil reservoir on RCP-ID due to some small leaks in the oil cooler.

All equipment was re_ paired.
