Dear Mr. Spencer:

This letter is in response to your letter of October 23,1980, which discussed the results of the inspection conducted August 29 to Septem-ber 26, 1980, of activities authorized by Nuclear Regulatory Commission Construction Permit Numbers CPPR-154 and CPPR-155. The letter identified two (2) items of noncompliance categorized in accordance with Reference 2 and required the Supply System to provide a response to these items.

The specific Nuclear Regulatory Commission Findings, as stated in your letter, and the Supply System responses are provided in Attachment I of this letter.

I Nuclear Regulatory Commission - Finding A 10 CFR 50, Appendix B, Criterion V states, in part, " Activities affecting '

quality shall be... accomplished in accordance with... instructions, procedure, or drawings. . ."

Paragraph 17.3.5.2 of the quality Assurance Program documented in the PSAR states, in part, "C-E sup; liers are required to control and perform all activities affecting quality, including inspection, tests and special cco;-

cesses in accordance with written instructions, procedures, or drawings..."

Combustion-Engineering (C-E) scope of supply for Unit 3 included the gas decay tanks. The C-E supplier's drawing, Richmond Engineering Company No. D-76-344, entitled, " Gas Decay Tanks", specified the size, type and location of structural velds for the tanks.

Contrary to the above, on September 10, 1980 gas decay tanks (Serial Nos.

N2320.10 through N2320.90) contained structural welds which were not in accordance with the drawing in that the welds were undersized, improperly located, missing and welds contained unequal leg fillets, undercut and incomplete fusion.

This is an infraction (Unit 3).

Corrective Steps Which Have Been Taken Nonconformance Reports 12240 and 12248 were prepared to document weld discre-pancies on gas decay tanks. A stop shipment order was issued to the tank contractor and will remain effective until their QA Program effectiveness is assured.

Corrective Steps Which Will Be Taken To Avoid Further Violations No future shipments of Contract 3240-02 safety related equipment will be made without Supply System approval. This approval will be based upon upgraded vendor surveillance reports and/or a final UDPSS inspection prior to shipment.

The WNP-3/5 Vendor Quality Assurance Program is being strengthened to function more effectively. This new program will include specific provisions for review of recurring problems or deficiencies. Strong corrective actions have been initiated to control and correct adverse trends. All Ebasco Vendor Quality Assurance Representatives assigned to WNP-3/5 contracts will receive on-site indoctrination and training. Included in this training will be site-specific requirements, specific contract requirements and a synopsis of vendor fabrica-tion history, and quality problems.

Date Of Full Compliance The WNP-3/5 Vendor Quality Assurance Program will be fully implemented by flarch 1,1981.

Response to Vendor Quality Program Inquiry Your letter of October 23 requested a description of actions taken to assure the effectiveness of our Vendor Quality Program. The reinforcement and additional efforts being placed in this area were discussed in our response to Finding A noted on page one.

Nuclear Regulatory Conmission - Finding B 10 CFR 50, Appendix B, Criterion IV states, in part, that " measures shall be established to assure that applicable regulatory requirements, design bases, and other requirements which are necessary to assure adequate quality are suitably included or referenced in documents for procurement of material, equipment, and services..."

Paragraph 17.1.4 of the QA Program documented in the PSAR states, in part, that "...The WPPSS QA Program requires the review and approval of WPPSS and Ebasco generated procurement documents. These reviews verify the inclusion of applicable technical and quality requirements."

Table 3.2-1 of the PSAR specifies that the emergency diesel generators and the boric acid makeup pumps are Quality Class I.

Contrary to the above, as of September 15, 1980, Contract Specification No. 3240-251, entitled, " Erection of Piping Systems and Installation of Mechanical Equipment", a WPPSS/Ebasco generated procurement document, identi-fied the emergency diesel generators as Quality Class II. As of September 22, 1980, the same contract specification identified the boric acid makeup pumps as Quality Class II.

This is a deficiency (applicable to both Units 3 and 5).

Corrective Steps Which Have Been Taken The WPPSS Quality II classifications of the Emergency Diesel Generators and Boric Acid Makeup Pumps is currently being revised in Contract Specification 3240-251 by Owner / Engineer design change document to reflect Quality Class I, their correct Quality Class as delineated in Table 3.2-1 of the PSAR.

Corrective Steps Wi.ich Will Be Taken To Avoid Further Violations On September 18,19 and 21,1980 the Owner / Engineer reviewed Contract Specifi-cation 3240-251, Section 2A-D and its applicable Field Change Request (FCR) against Table 3.2-1 of the PSAR. No further discrepancies were found with WPPSS Quality Classifications for equip ~ent delineated in this specification.

Date Of Full Compliance Date of full compliance will be December 1,1980.

In addition to the steps identified above, all WNP-3/5 contracts which involve Owner furnished equipment will be reviewed by Ebasco Engineering to assure consistency with PSAR Quality classification designations. This review will be complete by January 15, 1981.
