APPEHDIX A Washington Public Power Supply System P. O. Box 963 Richland, Washington 99352 Docket Nos. 50-508 d0-509 HOTICE OF VIOLATION Based on the results of an NRC inspection conducted during the period of August 29 - September 26, 1980, it appears that certain of your activities were not conducted in full compliance with conditions of your URC Facility Construction Permits Nos. CPPR-154 (Unit 3) and CPPR-155 (Unit 5) as indicated below.
A. ' 10 CFR 50, Appendix B, Criterion V states, in part, " Activities affecting quality snail be...accomplisned in accordance with... instructions proceaure, or crawings..."
Paragraph 17.3.6.2 of the Quality Assurance Program documented in tne PSAR states, in part, "C-E suppliers are required to control and perform all activities affecting quality, including inspection, tests and special processes in occardance with written instructions, procedures, or drawings.... "
Combustion-Engineering (C-E) scope of supply for Unit 3 included the gas decay tanks.
The C-E supplier's drawing, Richmond Engineering Company do. D-75-344, entitled " Gas Decay Tanks" specified the size, type and location of structural welds for the tanks.
Contrary to the above, on Septamoer 10, 1980, gas decay tanks (Serial Hos. 32320.10 through H2320.90) contained structural welds which were not in accordance with the drawing in that the welds were undersized, improperly located, missing, and welds contained unequal leg fillets, undercut and incomplete fusion.
This is an infraction (Unit 3).
B.
10 CFR 50, Appendix B, Criterion IV states, in part, that " measures shall be established to assure that applicable regulatory requirements, design bases, and other requirements which are necessary to assure adequate quality are suitably included or referenced in documents for procurement of material, equipment, and services...."
8104240I5(319
. Paragraph 17.l.4 of the QA Program documented in the PSAR states, in part, that "...The WPPSS QA Program requires the review and approval of WPPSS and EBASCO generated procurement documents. These reviews verify the inclusion of applicable technical and qua-lity requirements...."
Table 3.2-1 of the PSAR specifies that the emergency diesel generators and the boric acid makeup pumps are Quality Class I.
Contrary to the aLove, as of September 15, 1980, contract specification ilo. 3240-251, entitled " Erection of Piping Systems and Installation of Mechanical Equipment", a WPPSS/EBASCO generated procurement document, identified the emergency deisel generators as Quality Class II.
As of September 22, 1980, the same contract specification identified the boric acid makeup pumps as Quality Class II.
This is a deficiency (applicable to both Units 3 and 5).
