O' APPENDIX A NOTICE OF VIOLATION Alabama Power Company License No. CPPR-86 Farley Based on the NRC inspection October 14 - November. 5, 1980, certain of your activities were apparently not conducted in full compliance with NRC require-ments as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50 and implemented by paragraph 17.1.5 of the FSAR, " Activities affecting quality shall be prescribed by documented instructions, procedures....
Instructions, proce-dures... shall include appropriate.... acceptance criteria for determining that important activities have been satisfactorily accomplished."
Contrary to the above, on October 14, 1980, the procedures for the control of PSI /ISI (FNP-0-AP-57, revision 1 and "Preservice Inspection Program" procedures) were inadequate to insure proper evaluation and disposition of NDE inspection results, resulting in the licensee reporting by letter dated September 22, 1980 that all PSI indications in class 1 and 2 components had been dispositioned when in fact NDE indications in 17 welds / components had' not been dispostioned in writing.
This is an Infraction.
