Page 1 of 1 Docker No. 50-339 Report No. LER 80-082/03L-0 Description of Event On November 25, 1980 while performing a Degraded Voltage Test, it was discovered that the breaker 24J1-2 for the inside recirculation spray pump failed to open upon receiving the undervoltage test signal, and consequently 2-RS-P-1B was inoperable until the breaker could be repaired. The applicable T. S. is 3.6.2.2, and the event is reportable pursuant to T.S. 6.9.1.9.b.

Probable Consequences of Occurrence Since the breaker was repaired and the pump returned to an operable status in 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br />, and Train A was operable, the health and safety of the public were not affected.

Cause of Event The failure of the breaker to open was due to a stuck linkage arm that prevented a tripping latch to fall into place.

Immediate Corrective Action The immediate corrective action was to declare 2-RS-P-1B inoperable, and issue a maintenance report to repair the breaker.

Actions Taken to Prevent Reccurrence The corrective action was to clean and lubricate the trip linkage in the breaker and retest to verify the breaker performed satisfactorily.
