Description of Event

While operating in hede 5, an 18 month recalibration test was performed during which a flow transmitter, FT-2415, was discovered to be 2.39% higher than required by T.S. 3.3.1.1 in order for it to initiate a reactor trip at 89% of the design flow rate.

Probable Consequences of Occurrence Since the transmitter was recalibrated immediately, and there were.

two redundant transmitters in the A loop to trip the reactor on a loss of flow, the health and safety of the public were not affected.

Cause of Event

The cause of the event is due to instrument drift.

Immediate Corrective Action

The immediate corrective action was to recalibrate the transmitter and verify that is was set properly.
