- Description of Event On November 1, 1980, the 2H' diesel room fire door had been non-func-tional for. greater than a seven day period. This~ door had been declared non-functional on October 24, 1980 and it was replaced November 1, 1980.

This special report is required by the Action Statement of T.S. 3.7.15 since the fire door was not functional within a seven day period.

Probable Consequences of Occurrence A contin:'ous-fire watch was posted in the turbine building passage

[

- outside of the 2H diesel room within I hour after it was declared non-func-4

- tional as required by the Action Statement.

As-a result the health and safety of the general public were not affected.

Cause of Event

The door-had been deformed by repeated use while the room was under vacuum from air induction into the diesel during operation. The door opened against the force of air rushing into the room. When it was released, it would. slam shut and eventually the door, hinges and striker plate were deformed until it fail to seal properly.

- Immediate Corrective Action When;the door was' declared non-functional, a fire watch was posted.

and a maintenance report was submitted. Subsequently the door and required.. fittings were replaced.

Scheduled Corrective Action There are no scheduled corrective actions.

Actions Taken to Prevent Reccurrence No-action is required.
