Identification an'd Description of Occurrence:

Flow bias circuitry of APRh high flux began to drift high and was declared inoperable at 0100.

Conditions ' Prior to Occurrence:

U-1 at 96% power U-2 refueling U-3 at 97% power Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

Returned to service within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> Apparent Cause of Occurrence: The RHR heat exchanger head was removed and the lower head used as a catch basin. The RHRSW valves were leaking enough to over-flow the basin. The water ran 'down on panel 25-7 into transmitter FT-68-5A(5B) shorting the signal leads. This caused flow bias to drift upscale.

1 Analysis of Occurrence:

3 There was no damage to plant equipment. There was no activity release, no personnel exposure or injury and no, dange,r to the health or safety of the public.

Corrective Action

The transmitter was drained and dried. The flow bias was functionally tested and returned to service at 0845.
