YA APPENDIX A NOTICE OF VIOLATION Duke Power Company License No. CPPR-84 MCGuire Based on the NRC inspection June 16-19 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by 10 CFR 30, Appendix B, Criterion V, and implemented by DPC Topical Report Duke 1A Section 17, Paragraph 17.1.5 specifies in part that,
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances...".
Contrary to the above, ultrasonic examination procedures ISI-120 Rev. 8 and ISI-131 Rev. 5 used for examining saiety-related piping welds and reactor vessel welds, respectively, in preservice inspections do not provide adequate instructions for performing and recording the examinations.
This is a deficiency.
