During performance of SI 4.2.B-45 a diesel generator voltage available relay was f'ound dropped out,all the time. During LOCA and loss of offsite power this could have allowed RIDL 3A breaker go close on a dead bus, trip and auto-initiate locke.ut.

Open wire in cable caused relay to be dropped cut. Cable was apparently damaged during modifications which added cables in a penetration to the spreader room.
