Tennessee Valley Authority Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/80188 Technical Specification Involved: 3.5.5 Reported Under Technical Specification: 6.9.1.13.b Date of Occurrence: 11/17/80 Time of Occurrence: 1830 CST Identification and Description of Occurrence:

Chemical analysis determined that the refueling water storage tank (RWST) boron concentration was 2149 ppm. LCO 3.5.5, which requires a boron concentration between 2000 and 2l00 ppm, was entered.

Conditions prior to Occurrence:

Unit in Mode 1 at approximately 507. reactor power.

Apparent Cause of Occurrence:

A laboratory sample of 2009 ppm which was used as a basis for recommending an addition to the RWST boron concentration in order to increase the concentration to the middle of the allowable range was incorrect. The recommended addition to the concentration was therefore calculated based on an incorrect sample.

Analysis of Occurrence:

The sample which indicated a concentration of 2009 ppm was incorrect, apparently due to a non-representative sample being obtained due to tank stratification.

Corrective Action:

10,000 gallons was removed from the RWST and replaced with 10,000 gallons of boron free makeup water in order to restore the concentration to within the acceptable range. The boron concentration was restored within limits within the time limit of the LCO.

The sampling method for the RWST has been changed to give a representative sample of the tank contents.
