Description of Event

On October 31 with unic 2 at 91% power it was discovered that the required 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> survex11ance (2-PT-26.3 - Axial Power Distribution Monitoring) had exceeded the surveillance interval by 5. " hahrs.

This

.urveillance was required because the Axial Power Distrib.

c Monitoring System was inoperable at the time.

Probable Consequences of Occurrence The consequences of this event were minimal because the periodic test was immediately performed and the core axial power distribution was within the. limits of T.S..s3.2.6.

Consequently, the health and safety of the public were_not affected.

Cause of Event

The individuals responsible for performing the test over looked thi fact that the test was required to be performed every eight hours.

Immediat'e Corrective Action The procedure 2-PT-26.3. (Axial Power Distribution Monitoring) was performed with satisfactory results.

Actions Taken to Prevent Recurrence

The control room log was changed to include an APDMS check every four hours for operability and evaluate the need to perform 2-PT-26.3.
