O APPENDIX A NOTICE 0F VIOLATION Philadelphia Electric Company Docket No. 50-277 Based on the results of an NRC inspection conducted on September 1-30,1980, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-44 as indicated below. This item is an infraction.
Technical Specification 3.7.C, Secondary Containment, states in part, " Secondary Containment Integrity shall be maintained during all modes of operation..."
Contrary to the above, on September 23, 1980, with the Unit 2 reactor in power operation, secondary containment was breached for about two minutes at the northeast cirlock entrance to the Unit 2 Reactor Building by blocking the inner Secondary Containment noor open and then opening the outer Secondary Containment door to remove scaffolding material from the Unit 2 Reactor Building.
