March 20, 1978 Facility: Oconee Unit 3, Seneca, South Carolina Identification of Occurrence: Valve 3HP-24 Inoperability Conditions Prior to Occurrence:

100% Full Power Description of Occurrence:

At 0924, on March 20,.1978, during the performance of PT/0/A/0310/12A &l2B (monthly ES test), Valve 3HP-24 failed to operate on manual actuation from the control room. At 0945, during subsequent investigation of the incident, a second attempt was made to open the valve and it functioned properly. The contacts in the valve operator were cleaned and the valve was cycled to verify operability. At 1030, the valve was returned to service.

Apparent Cause of Occurrence:

The cause of the failure was foreign material on the contacts of the valve's operator.

, Analysis of Occurrence:

Technical Specification 3.3.2(b) requires the operability of all Engineered safeguard valves related to the HPI System, but Technical Specification 3.3.5 allows up to 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> to repair inoperable components (with some exceptions).

Valve 3HP-24 was inoperable for a maximum of 66 minutes and operated properly without any corrective action shortly subsequent to the initial failure to function.

It is considered that this incident did not adversely effect public health and safety.

Corrective Actions

Valve 3HP-24 was examined. repaired and returned to service.
