This refers to your letter dated September 15, 1980, in response to

our letter dated August 20, 1980.

Thank you for the explanation documented in your letter concerning

the item of noncompliance which we cited. By your own admission, the

situation arose when the attention of the Group Shift Surtrvisor was

diverted by other operations in progress thereby causing him to fail to

implement the verbal directive. We view the fact that distractions

such as " operations in progress" could cause the failure to implement

safety significant directives for a period of 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> to be indicative

of an inadequate mechanism for insuring management instructions are

followed. Therefore, we find the item of noncompliance to stand as

cited. We understand, based on conversations between Mr. Briggs of

this office and Messrs Carroll, Sullivan and other members of your

staff on September 25 and October 7,1980, that Procedure 106 will be

revised to formalize the Operational Memos and Directives notebook

which will be used to issue management directives that are of short

term applicability and that this notebook will be required reading,

with appropriate sign off, for each shift and operating supervisor at

each shift turnover. We also understand that the above action will be

completed by October 24, 1980. These actions will be examined during a

subsequent inspection of your licensed program.
